in men growing breasts the lower lobe/hemisphere over the fold is usually under developed resulting in the areola and nipple appearing to be too low on the breast? is this inevitable without implants?

Just the natural body structure. I think that the same would be true if you were to compare a totally flat-chested female to a regular male: the male's nipples would be lower.

That's just what I recall however, I might be incorrect.
