Is it possible for a curse file to take effect from the first time a person listens? And some say irreversible after the first time a person listens.

Yes, I believe it's possible. Especially if you really desire what the curse is about and the script manages to convince your mind that there's no going back.
A suggestion absorbed by the unconscious can keep having effect on you long after you stop listening to the file (I think i know something about this).
The saying “beware what you wish for because you might get it” is appropriate.
