Trouble uploading file

I've tried to upload a file for the past couple days. I get a message that upload was successful, but then the upload form fills in with information (that is not my file) and the file is not available on the site.

Try changing the name, otherwise see if you can get a couple screen shots and send them to admin@warpmymind.com so I can figure out what's going on.

Tried a new file name and was successful.
